can we include a check box field for selecting multiple values in record producer? what should be the variable type in record producer and field type in form?

That works. Can I group all these fields under one header in form?

Like X,Y,Z and other under platform in form.

I dont think you can group them together as on Record producer. But close to that, you can add them in a column, and if you need, you can add annotation to the group.

Hi Brad, is there a way to use the Multiple Choice "radio buttons" on the Incident form?  

I'm looking to create something like we have on HI when we report an issue

I only see T/F field in form design
