Create Incident form on Service Portal shown twice?

Hi all,

a customer of mine has a problem with the Create Incident Catalog Item in the Service Portal. THe Incident form from the record producer is show twice. Anyone any idea? 

Funny thing is, it works correctly on the DEV Instance, but not on TEST amd PROD. Don't now and can't find out what they changed last time I was there 😞

Hi,

May be page has 2 widgets added. Open the page in page designer and check  for the widgets added to the page.

Ctrl+Right click on the page and select 'Page in Designer' to see the widgets added to the page.

thanks for your quick reply. Which page is that? This is a Catalog Item and I can't find a page to check?
Aren't these pages built "automatically" by ServiceNow?

Sorry for this Dummy question, but I'm not a Service Portal guy

OK, thanks again! found it and you were right, the OOB widget and the customized widget was shown on the page
