duplicate entries in activities and approval history for flow ask for approval action

When using the ask for approval action in flow, two entries are generated in the custom table's approval history and activities fields. This table is an extension of the tasks table. If I ask for the approval manually using the related list, this behavior does not happen. Maybe something I'm doing wrong in the flow action?
Here are the duplicate entries in the approval history field (I had two approvers)

Just check the trigger condition for flow.

i believe it is due to some BR getting triggered after the flow or during the flow execution.

Thanks Ashutosh. I'm not sure how to go about debugging this. I tried debugging Business Rules but nothing came up (though this was a session debug so maybe because the approvals are running a flow it does not log it to the session debug?) . A couple of other things I noticed:
- If I use an Ask for Approval action on another custom table that is not an extension of the task table, the duplicate problem does not happen.
- The event log shows two events for the approval history:
- Mark as New
- Bookmark
- Subscribe
- Mute
- Subscribe to RSS Feed
- Permalink
- Report Inappropriate Content
