Implement new Problem process flow

Hello,

 

As per the attachment, I have been asked to implement new problem process flow within the Incident, While clicking the "Create Problem"button and filling in all necessary information while raising problem ticket/form. I need to add 3 buttons/options such as 1) Approve, 2) More Information required and 3) Reject. As soon as I define these 3 options the next steps should be 1) After Approval -> Problem ticket should assigned to Problem management team, 2) More Information required -> Awaiting Info Caller should be notified via email for pending details, 3) On Reject -> Problem rejected- and closed with Problem managment Team with notification. 

 

I need solution on this If we can acheive from Script Include and steps ? Urgent
