Hi there,
We are noticing a strange behavior (currently only in our DEV instance, phew!) with some Requested Items.
They are generating two workflows!
We created a custom form for New Hire request, that workflow is fine, from there we call other workflows, for example, new laptop, email account creation, software request, application access, etc.
Everything was running as expected since 2018, but the last few days I noticed this strange behavior in DEV.
My initial thought was something related to Utah Path 7 that was installed last weekend, but in our TEST instance, updated to Utah Patch 7 in the same weekend as DEV, this issue doesn't happen.
So, it might be something in development yet, but we have not found anything so far.
Anyone have any clue?

Hi,
Navigate to Workflow > Administration > Workflow versions
Filter by Table = Requested Item
Search with the Workflow name ATC - Access Request. There should be one entry which should be published (published = true)

That's fine here, we have only one published version of each.
