Help in creating GMUD table

I have to make a GMUD application, but I'm confused about how to create the table and what fields I put in, and what these fields will be like, these are the application requirements:
- Create a new GMUD request
- Must contain a description of the change
- It must contain the list of approvals for this change
- It must contain the date of execution of the change
- Must contain change status control
- Must allow gmud to be sent to different execution and approval queues
Does anyone have any ideas on how to do it?

If GMUD is "management of software development change" then you can either use Change management which covers all of your requirements or you can use the SDLC app and change management to cover all of what you need.
To keep things simple I would just use Change Management since it has all of the things you need by default.

And if you have the roles the Application is just called "Change", it should be listed under All by default if you have the roles for it.
