How to add or remove fields on Service Portal

We want to remove the two fields highlighted in red and replace them with different ones.  Where on the widget can I do this?

OOTB Widget name: SC Order Status

OOTB widget id: sc_order_status

Please clone the widget "SC Order Status" and update the highlighted lines in html and server side scripting of widget as per your requirement
