Incident form "on behalf of" customisation return to OOB

How do I revert this back to out the box functionality. 

On the "Create Incident" form I have a check box which then brings up additional questions if selected. How do i get rid of this and revert back to Out the box functionality where it has the option "Raised on Behalf of" & then how do i ensure the email notifications are turned on so if raised "on behalf of" the person also gets the notification

You just remove it from the form and delete it from the record producer and put the OOB field on the form and create a variable to map to the field on the form. Then you add the field as recipient of the email on the notifications.

 

Again: we don't have access to you instance, so we don't know what kind of customization you have in place. We can't see what you have done or help you without sharing more information. If anything like scripts/ACL's/policies are added, we don't know about that, so you will have to check that yourself if you can't share it with us.

There can be all kinds of reasons for this but you aren't sharing any information that can lead us to the cause. Are you looking at the correct view? Did you check on client scripts? Did you check on ui policies? Did you check on ACL? You are only describing the result of something that you did. Once again: we don't have access to your instance, so you need to tell us what is going on.

Remove the field itself from both backend form and record producer. Delete the UI policies (or client scripts) that take care of making the fields visible, so they are always visible an for the email notifications I'm not sure what you are expecting? I assume they are already in place for when the field is filled?
Do realize that you are asking us about a custom solution that has been implemented on your instance without us having access to your instance. We don't know what has been done to get this working (and what has been turned off for this).

What i am asking is

1 - how do i remove the customisation of that specific field (Screenshots both in Form and Record Producer) 

2 - How to i add the out the box "Raised on Behalf off" on both the incident form and record producer 

3 - when i add this out the box functionality how do i enable notifications to go to all involved (requestor and the "on behalf of")

  I have managed to remove the field thanks, But when i add the OOTB variable back in for some reason on the form it does not appear, I checked via the form designer as well but couldnt identify the issue
