Hi Nataliia,
For the custom task table to appear in My Requests, you need to configure it from Service Catalog > My Request Filter. You will pick the table name, condition (e.g., opened for is dynamic me), portal page to open (e.g., ticket).
Then you configure Standard Ticket Configuration to tell what actually appears on that record (e.g., short description, due date).
