To create flowdesigner using Table

i need to create flowdesigner from scratch.can any one help me on fulfilling this scenario for the better practice

What have you created till now in the flow, please share .

flow designer is not need.i have a query that i have created Ui actions in above mentioned ouery but some are not working can you please help with this.

my requirement is 

1)when i click on awaiting response only Resume button need to show on form header

2)when i click on resume it need to show 3 buttons Awaiting response ,Downgrade, Declare crisis

3)when i click on Downgrade i need to show only Reopen button

This is my scenario can you please help with this
