Example - HR Service Catalog Intake Form

Greetings all - 

 

Some time ago someone shared with me a case intake requirements gathering document to be used when capturing any and all requirements with the business and COEs.  It was an excel / worksheet with multiple tabs that included details such as: 

  • Short description 
  • Long description 
  • Field Types / names 
  • Drop downs 
  • Visibility 
  • Assignment Groups
  • COE details 

I understood this to be an asset that SN provided but cannot find it anywhere.  

Anyone?
