Form Design is not saving changes

My Form design is not saving my changes. I can rearrange, add, and remove fields. When I hit save I get a pop up and progress bar. When the bar is completed it takes me back to the form designed will all my changes reverted.

I am on a Mac and tried it in Chrome, Safari, and Firefox.

I am using a dev instance of Fuji.

Thank you for the link. It was helpful to get me through that question. Outside of the exercise though, when I used the configure choices and added what I thought would be appropriate for my application, it's defaulting to the numerical number 1instead of to one of the options I input (Pending, Completed, Rejected)

Any thoughts?

Can you please provide more details on whether the choice field is a custom or it is extended field from task table.

This has been extended from the task table.
