How do I add a list of CI's when I create a change template. I do not see where I can do this. It would be the "affected CI's" tab.

They don't come on the form out of the box, so you have to use Configure -> Form Layout and add them.

Thank you Michael. Sorry, that is what I mean - when I go to Configure ->
Form Layout , I do not see 'Related Child Template' or 'Next Related
Template' to add. I am in Geneva UI16.
Also, I do not see the task_ci table when editing the template. Attached
below is a list of 'task' tables that I see (I can't find 'task_ci').
Thanks again for your help.

Hi Michael
I am also having the same requirement and tried to apply templates in Next Related Child Template or/and Next Related Template but it does not get populated.
Here is what I've tried:
1. Created a template3 on task_ci table and set CI as *Annie-IBM
2. Created another template4 on conflict table and set type as CI Already Scheduled
3. Inserted Template3 and Template4 in Server Reboot template as shown.
It only sets the fields value when I create a new change and apply Server Reboot upon it. Even if I try to insert parameters in planned start/end date(in order to change the conflict status and run BR's), it does not show up with the template parameters. Any help is appreciated.

I haven't used Templates in so long. I just tried what I was telling you to do and you're right - it doesn't work that way anymore. UGH! Even if it did, templates are a pain to use and modify because of their daisy chain.
I'd suggest you look at the following 2 solutions where you can use CI groups:
Capture change request affected CIs & impacted services related lists into a template
Group Configuration Items for Easier Management in ServiceNow - ServiceNow Guru
