Question on my request widget

Hello All,
I have a requirement related to my Request widget. In the OOB widget, currently three column fields are displayed. I want to add two more columns: Stage and Created.
Please guide me on what changes are required and which part of the widget code (HTML / Client Script / Server Script) needs to be modified to display these fields.
