I created a new change request workflow and for some reason when a user a creates a Change request from an incident the workflow stops and never goes to Approval. I have tested this by creating a NEW Change request and it goes through fine. Only when creating from an incident does it fail. What did I miss?

Check your workflow condition. When does it trigger? also when you create change from incident I would also check the state values.

Sounds like you might have a problem with workflow trigger conditions. When a change is created from incident, does it have any prefilled values that might interfere with workflow trigger condition? You can check this by navigating to the workflow and sorting the conditions like shown in below image.

I found the conflict. I changed the category field have "none" which made it stop working when created from an incident.
