2nd Approval in Workflow not sending mail notification

I have 2 Approval in my Workflow, Approval 1 sends an email to the Approvers and adds the Approvals to the Request Item.  When Approval 1 is Approved the workflow goes to the next Approval.  The Approvals are added to the Request Item but no email is sent.  The Approval Notification should be sent when the Approval is added, the state "Requested" should be the trigger.  I have swapped the Approvals around and the mail goes out but does not get sent for the 2nd Approval.

 

Any help appreciated.

I've managed to get this working by Clearing the Stage of the Approval.  It was originally set to Review Stage.

Ankur, the workflow is for Knowledge if that helps.

Where did you clear the stage of the Approval? Within the criteria of the notification? Or elsewhere?
