2nd Approval in Workflow not sending mail notification

I have 2 Approval in my Workflow, Approval 1 sends an email to the Approvers and adds the Approvals to the Request Item.  When Approval 1 is Approved the workflow goes to the next Approval.  The Approvals are added to the Request Item but no email is sent.  The Approval Notification should be sent when the Approval is added, the state "Requested" should be the trigger.  I have swapped the Approvals around and the mail goes out but does not get sent for the 2nd Approval.

 

Any help appreciated.

I've managed to get this working by Clearing the Stage of the Approval.  It was originally set to Review Stage.

Hi,

For the 2nd approval users/group -> Did you check these?

1) users has email field populated

2) users have notification preferences enabled

3) users are active and not locked out

Yes on all of those.  There is no issue with them receiving the mail when I swap the Approvals around.  The issue is that whichever approval is 2nd it's not sending an email notification.

Is this happening with all catalog items having 2 levels of approvals?
