unable to add the fileds in form builder,form design,form layout

in Form it is showing fields, when i want to add extra fields in to the form through form layout,
I am unable to see the fields in it but in form builder am able to see fields while adding fields in to the form it is not capture in the form. 

kindly give any suggestions.
