Do we have the data model of OM or any reference document which will guide us like in which table the attribute/characteristic/actions are getting stored?

The entire ERD is not available in public facing docs but there are some references in ServiceNow documentations -

(Utah - CSM OM)

Please note, going forward there will be one single converged OM product. (OM 6.0 onwards)

The schema otherwise can be explored from within a ServiceNow instance using Schema Map.
