Why do we see OOTB records shown as Skipped Records post upgrade?

Recently we are seeing many OOTB records (untouched by us) appearing in the Skipped Records.
As far as I noticed, most of them were records updated by a Patch or Hot Fix.
Here is a good example, last year, ServiceNow released this KB:

If you see the change log in the KB, it matches the updated time in our base system record:
October 26th 2023, one day prior the published date.
And even though, we never touched that widget, this is what we saw in our skipped records after upgrading to Washignton DC:
Anyone else seeing this type of thing?

@Luiz Lucena Yes, such skipped records are quite common and often observed during upgrades. As rightly mentioned by you such customisation get introduced during a hot-fix or a patch by ServiceNow.
It is a best practice to usually compare such skipped records with the OOTB version and see if the customisation can be reverted back to base system.

I've opened a NOW support case as they are the only ones that touched that record, there is a difference in the Server Script.

@Luiz Lucena They have added some ACL related checks in the customised record. This may be related to a security vulnerability they identified in Simple list widget a couple of months back they also released an advisory for it here is the link for the same https://support.servicenow.com/kb?id=kb_article_view&sysparm_article=KB1553688
