Display change request on Requested item form

I have requirement for ITSM. 
on the requested item form we have section called related records which is OOB feature . 

I need to display change request lookup under related records section on requested item form.

I tried in below way,

created a new reference field change request and tried adding under the related records section using form layout ,but it was not taking as reference instead taking as a list.

If anyone has any idea please let me know.

What's the relationship between Change and Requested Items in your org? What are they hoping to achieve from this?

Once request is submitted, the change request need to be present if needed we can add it.

Hello,

You can configure the related lists on the requested item form.

1. Right click on the header of the requested item form

2. Configure -> Related Lists

3. Bring "Change Request -> Parent" to right side

4. Save

5. You will see the "Change Requests" related list and you can relate any number of changes here to the requested item
