How to input values of fields inside Description of an incident?

I am filling up the values of fields while raising an incident and then I want to see few of those field values inside description of an incident form. 
Please assist.

 I am filling up 6 fields via a record producer and submitting which is creating an incident . 
Now from those 6 fields , i need 3 of those values to be displayed in the description like LCP Component, Instance ID, Cloud Region name ..
How to do that?

You can configure in the Record Producer in the script using similar logic in my another reply.
