Survey Designer and Attestation Designer are very similar. The Attestation in GRC is an assessment not a survey.
You can search in the community and find a couple of articles about the difference between surveys and assessments.
The big difference between surveys and assessments is that assessments generally resolve to a number. Questions are asked and those are answers are related to values and then there is an overall value assigned to the assessment. Surveys don't transform the answers into values.
The Attestation designer is built on the baseline functionality of the survey designer. When you create a control attestation it populates some fields in the background to identify it as a control attestation. It also has some default questions that are populated.

Attestations typically require users to review and confirm their adherence to certain rules or acknowledge company policies.
To create an attestation campaign using the Attestation Designer, follow these steps:
- Go to "Compliance > Attestation Designer" in the ServiceNow instance.
- Click on "Create New" to start defining a new attestation campaign.
- Configure the attestation by specifying the applicable policies, users, and frequency.
- Customize the content of the attestation message and set the completion deadline.
- Save and publish the attestation campaign to notify users and initiate the acknowledgment process.
