HR Service Case- OOB Notification not triggered after submitting through portal.

I am sending a new HR service request through the portal. A notification should be triggered to be sent to the user for whom it is opened for, for which I am using an OOB notification. But it is not getting triggered.
The conditions used are "active" is "true".

Did you check for that user?
1) user is active and not locked out
2) user has email field populated on record
3) user has notification preferences enabled for his account

I have checked it and the user is active and has email populated on record, the notification is enabled as well.
It is working on OOB Coe like HR core case but not triggered on custom COE's.
We also tried to make new notifications but the Custom COE is not getting showed on the table field.
