Incident fields missing information or grayed out

Hello Everyone, quick question "How do I fix an issue with incidents > create new > i fill out all the required fields and those that are supposed to be auto populated do populate , however when i click save on the ticket it creates the incident but a couple of fields such as CI, support group, category or subcategory is not populated after the incident is created, same issue happens with self service tickets, 

 

1. self service tickets get created but is missing fields such as ci, support group etc, or those fields comeback grayed out same happens with the newly created tickets before clicking save they show all fields populated however once saved and we get the incident number we open it and its missing fields information or they are just grayed out

It could be access control related. I would enable "Debug Security Rules" and review the logs to see whether there is anything preventing those fields from being populated.
