Work Notes and Comments are not displaying on activity

When I enter work notes or additional comments it is not showing in the activity. What might be the issue?

2.  Does this happen to everyone?

3.  Does this only happen on the incident table?

I just noticed this.

2.  Does this happen to everyone?

This is in my developer instance.

3.  Does this only happen on the incident table?

yes, this is only on Incident table

Does it happen when you impersonate or log on with someone with a different role?

Yes, it does happen when I impersonate others.
