Work Notes and Comments are not displaying on activity

Hello Everyone,
When I enter work notes or additional comments it is not showing in the activity. What might be the issue?

Hello,
My co-worker found that I'm missing records in the dictionary for the incident and I'm trying to export and import but it's not letting me do that.
Any help would be appreciated.

Please try to check if there is record created in sys_journal_field
if yes, then try to enable audit on the table .
