Creating a Table within a form

Hi,
Within my change form I need to add a table, something like the below. Is this possible? and if so how can I do this?
I have try to create text table but that just does not work. Your ideas and suggesstions are welcome.

Hi Riaz,
what type of information do you need to view?
Wouldn't it be possible to use a related list instead?

Thanks Alex,
It needs to be edited, so similar to an Excel Table. It is for Firewall changes so something like the below.
This is from Excel. The idea is to add this table into a description box then people can just fill out the details.
Hope that makes sense. I just cannot find a good way of doing this.

Its not perfect, but allow you to add Table into a form.
