Update Record Action is not working in Flow Designer

Hi,

I am trying to cancel requested item when catalog task is set to cancelled from the flow designer as below, but this is not working. In executions, I do not see any output value. What might be issue here ? I am breaking my head since two days but couldn't able to figure it out. Please help me to resolve this issue. Thank you.

May be check below three cases

1. check the flow runs by system or who ever initiated the session.

2. If it runs by user who initiate the session, please check any  ACL/Business rule(update,query rule)
3.Please verify any Data policies making any fields mandatory when you change the status.
