Help with Upgrade Plan to upgrade the instance to Utah

Hi, 

I have issues with the Upgrade plan. When I built the Upgrade plan, received the following errors. 

1) Global Customizations are not added due to 'Duplicate application name found in application files'

2) Version already exists - Please update the version.

3) Company or Customization ID already exists in a different combination

4) Instance doesn't have access to base app

 

Has anyone experienced the same issues and how you managed to fix it? Thank you in advance.

I got the first error.  What i had to do was go into the global customization application files.  Search for the file and delete one of the duplicates.
