Hi Team,
Is there any way to export an excel template for Risk Assessment Designer to be used for creating set of questions? Is there any way to Import a set of questions (an excel file) to Risk Assessment Designer?

Thank you so much for you response, but this is not what I'm looking for.
This is what I'm asking, how can I export a template from Risk Assessment Designer then import the excel file created from that exported excel file? Is there a way to do that?

If my understanding is right, what you're trying to do is, to let the attestation respondent to submit an attachment file as sort of evidence.
"Attachment" control would meet your requirement. When you define the attestation questionnaire using "attestation designer", you've to add this control just like other controls like label, input box.
This control lets the respondent to upload the excel file.
And, to share any excel template to the respondents, one can use the OOB "Manage attachments" icon available on every record.
