how to send a survey email notification and get the response from the recipient?

Hi,

 We have a requirement where we need to send an email notification which contains a survey .once the user provides any response we need to get that response. for the reference I have provided an example survey format. please provide any solution to implement similar kind of email notification in ServiceNow.
