Is it possible to get choice answers from a survey to appear on a database view?

I am building a database view to be used to show survey answers in a report.

When I look at the table or the report, It shows the answers in the survey for all free text boxes but it shows all choices from the survey answers as "empty" 

I have looked through forums but have been unable to find anything on this issue as it seems to only be stopping survey choices from being viewed

I have attached screenshots of the issue within the table, how I have set up the database and one of the assessment instance questions within the database that should be displaying an answer from the survey choice
