Survey-based questionnaires
Survey-based questionnaires are the foundational questionnaire type in Field Service Management. Administrators can create question sets using the Survey Designer tool and associate them with work order tasks. When the configured trigger conditions are met, the questionnaire becomes available to the assigned agent directly from the task form.
Survey-based questionnaires support various types of questions and checklist items, and can be configured to be required before an agent closes a task.
Survey-based questionnaire use case
Marco is a field service technician assigned to a high-voltage electrical repair at a commercial building. The organization requires Marco to confirm that the site is safe and that the right protective equipment is in place.
When Marco's task moves into progress, a safety checklist in the form of a survey-based questionnaire automatically becomes available. Marco works through a fixed set of questions — whether the power supply has been isolated, whether the area is cordoned off, whether protective gear is worn — and submits the responses. Because the administrator has made this questionnaire mandatory, Marco can't close the task until it is complete.
The responses are recorded against the task, giving the organization a consistent, and auditable record of safety compliance across all similar jobs.
