How to get a list of incident created by current login user in to the widget

Create as simple list widget to fetch current user’s incidents.(Incident number and short description, State)When click on incident number it should open in 'ticket' form.

You can configure your inserted simple list widget's instance options like this (I have some more secondary fields added):

but the incident has to be open in a tickect form when i clicked the number
