to create new header item on service portal similar to my requests on portal

hi I am trying to create a header item for new table (which is similar to Incident) on service portal similar to my requests on portal.
I have tried to clone the my requests widget but didn't work. can anyone help me with the widget?

what does header item mean? Why do you need to clone My requests widget and what didn't work? Could you maybe describe your use case with screenshots? 

Maybe a simple configuration change will be sufficient?

these are menu items, so when we click on requests it shows our requests and incidents in the portal view.
tried this widget

to achieve my requirement but didn't work.
