This is not about reporting PDF, this is a custom PDF template I have to generate pdf with the help of UI action on the form.  

I have used below custom module

Hi

 All header and footer text uses 8-point Helvetica bold font. A PDF page header footer template is made of multiple cells containing report attributes or user-specified content.

Steps:

• Navigate to Reports> Header Footer Templates.
• Click New.
• Enter a Namefor the template.
• Select the content option for each header and footer cell, and enter or upload content as appropriate
