Need to add Contact number field and URL field in the UI Page

Hi Every one ,

I have a requirement that I need to add two fields one is contact number field and Teams Url type field in UiPage , and I  need to map this one to Comm-plan table fields.

Please share me the code how can I map this one. How can I add this fields in UI Page.

Hi Maik, 

The thing is in UI Page i need to add 3 fields, one field is contact number and second one is URL type field and third one is String field.hwre I need a script to add those fields.

After that I need to map those values to some other table called it as communication plan
