Request a vaccine exemption

Request a vaccine exemption from your organization with information about why you require an exemption.

Before you begin

Vaccine exemptions must be enabled by your organization to submit vaccine exemption requests. If you do not see the Request vaccine exemption module or you are unable to submit vaccine exemption requests, refer to your administrator to enable this feature.

Role required: none

Procedure

1. Navigate to All > Vaccination Status > Request vaccine exemption.
   Note:

   To request a vaccine exemption in the Now Mobile app, navigate to Health > Follow These Safety Practices > Request Vaccine Exemption.
2. On the form, fill in the fields.

   Table 1. Request Vaccine Exemption form

   Field	Description
   What is the vaccine exemption type?	The type of exemption you are requesting. Available values are religious, medical, or other.
   Details for vaccination exemption request	Provide any useful additional details about the request for your organization.

3. Optional: Add any relevant attachments to the form.
4. Click Submit.
5. Optional: Review the status of your vaccine exemption request by navigating to Health > Follow These Safety Practices > Vaccine Exemption Requests in the Now Mobile app.

Result

Your vaccine exemption request is submitted to your organization for an administrator or a manager to review.
