The Health and Safety case form enables safety case managers to manage and resolve health and safety questions submitted by employees. Each submitted question automatically generates a safety case with some fields pre-populated from the original request. Assigned safety agents use the Health and Safety Workspace to complete and update the case details efficiently.

- Compensation Claim Case: Option to flag cases associated with compensation claims, editable only by users with the Claim case writer role. Sections include employee claim details and closure information.
- Assignment and Contact: Cases are assigned to safety assignment groups and individual users for resolution. Contact type information helps determine how to communicate with the employee.
- Detailed Description: A comprehensive description field captures all relevant case details for effective case management.
Return to Work Details
This section appears only for cases marked as return to work cases and is editable by users with the appropriate role. It facilitates assessing an employee’s readiness to return by capturing:
- Leave category indicating the reason for absence.
- Expected return date.
- Whether medical clearance has been received before returning.
- Any accommodations needed to support the employee’s return.
- Work restrictions identified to ensure safe reintegration.

Safety case manager can use the safety case form to resolve health and safety questions that they receive from employees.
When a health and safety question is submitted, it creates a safety case. Some of the fields on the case form are populated from the request from which it is created.
After the case is assigned, a safety agent fills out the fields on the case from Health and Safety Workspace.

Return to work details
The Return to work details section on the Health and Safety case form appears only when Yes is selected from Return to work case. The fields enable you to assess the employee's capability to work before they return from an injury or illness.
Only a user with the Return to work case writer [sn_hs_cm.rtw_writer] role can edit fields in this section.
